"My child needs to take a prescribed medication during the school day, what do I need to do?"

In order for the school nurse, designated staff or student to safely administer a daily, over the counter or as needed medication to your child during school hours, the following must be completed:

 

1. Medication administration form must be completed and signed by the physician and parent/guardian. A medication administration form can be downloaded on this page or can be picked up in the nurse's office.

 

2. Medication must be labeled with the students name, medication name, dosage and instructions for administration. Dosage and instructions must match the signed physicians order.
 

3. Parent/guardian must drop form and medication off to the designated school nurse. ** Students may NOT transport medications to and from school.

4. If the ordered medication is to be carried and administered by the student out side of the nurse's office, the physicians order must state that the student is permitted to carry and administer said medication. ** This is not recommended at the Elementary School level. 

 

IMPORTANT: Please note, if there is a change in dosage or instructions of the medication that is prescribed to your child, a new form with the new dose and instructions must be completed and signed by the physician. Also, medication must be labeled with the new dose and instructions.
